The specter of illegal fentanyl looms over Costa Rica, with authorities confirming seven seizures of the synthetic opioid, 50 times more potent than heroin, in the past two years. This stealthy invader is responsible for claiming 110,000 American lives annually, standing as a testament to its lethal potency.
The Insidious Rise
According to the Judicial Investigation Agency (OIJ), four seizures were conducted in 2022 and three more in the current year, confiscating an equivalent of 1,000 pills. The investigations underway are closely guarded, leaving the origin of the seized fentanyl — whether internationally trafficked or locally produced — veiled in secrecy.
The Lethal Attraction
Fentanyl’s allure for the underworld is multifaceted. Unlike cocaine or heroin, its production does not demand extensive coca and poppy plantations. With a minimal investment of $800 in chemicals sourced from China, traffickers can manufacture approximately 415,000 fentanyl pills, yielding substantial profits.
A Double-Edged Sword
Fentanyl is a dual-faced entity. While pharmaceutical fentanyl is a legitimate medical substance used for severe pain management, its illicit counterpart, often mixed with precursors and various other compounds like cocaine, methadone, heroin, and amphetamines, is the harbinger of the current overdose epidemic.

Legal Yet Dangerous
Though fentanyl does have legitimate medical uses within the Costa Rican Social Security Fund (CCSS), its administration is strictly controlled and monitored, said Heldren Solórzano, Deputy Director of the Costa Rican Drug Institute (ICD).
From Euphoria to Death
Fentanyl induces a state of heightened ecstasy within seconds, followed by a gradual descent into drowsiness and sedation. The risk of overdose and death is significantly high, with traffickers often experimenting by mixing fentanyl with other drugs to enhance its effects.
Cartel’s Favorite
Fentanyl’s characteristics — easy transportation in pill form and simpler production processes — make it a favored choice among traffickers. Its production does not require specialized labs or highly skilled personnel, making it accessible for production with basic knowledge.
Health and Social Implications
Mauren Acuña, a psychologist at the Institute on Alcoholism and Drug Dependence (IAFA), highlighted the severe and concerning problems caused by opioid consumption, with the rising use of fentanyl in Costa Rica potentially leading to significant economic, social, and health repercussions.
A Grave Economic Challenge
The rehabilitation and recovery of fentanyl users will also pose an economic challenge to the country, necessitating investments in specialists and interdisciplinary teams dedicated to aiding fentanyl users. Acuña stressed that anyone could develop an addiction, urging those in need to seek help, guidance, and support.
Unseen Dangers
The risks of fentanyl are not confined to its users alone. Producers, handlers, and even those involved in its confiscation are at risk if safety protocols are not meticulously followed. Improperly formulated pills can be fatal upon contact.

The OIJ is bracing for the challenges posed by the spread of fentanyl in Costa Rica. Special protocols, including the use of specific equipment and gloves, are imperative for handling the drug safely, underlining the gravity of the issue and the need for serious attention and action.
